Could Two Bald Spots at the Base of My Dog’s Tail Be Mange?

It could be, but bald spots at the base of the tail in an itchy dog are most often caused by fleas. Many dogs are allergic to flea saliva, and just a few bites can trigger intense itching and chewing right where the tail meets the back. The two matching patches may simply be where she can reach best when she scratches or chews.
Other possibilities include skin allergies, a bacterial skin infection, and, less often, mange. Sarcoptic mange usually starts on the ear edges, elbows and belly, while demodectic mange tends to begin on the face and front legs, so tail-base patches are less typical of either.
Check her closely for fleas or flea dirt (tiny black specks that turn red on a damp paper towel), and keep every pet in the home on a vet-recommended flea preventive. If the itching is constant, the spots are spreading, or the skin looks red or sore, have your vet take a look. A quick skin scraping can check for mites.
