Dog With IBD That Isn't Responding to Treatment

Reader Question
Help!!! My 8 yr old pit bull/lab Lily was diagnosed with IBD from an endoscopy done in 2011. She's on budesonide, metronidazole, prilosec and now entero pro biotics and NOTHING controls the inflammation. Cerenia and odansetron do not control nausea either. No medications are working!!!

After continuous vet visits and thousands of dollars, she is worse than ever. Ultrasound in January shows abnormality in small and large intestine, stomach and pancreas. Inflammation from ultrasound last january was only in small intestine and stomach.

She has tried royal canin hypoallergenic hydrolyzed food and is now on the ultamino food. Even tried tilapia and sweet potatoes and she threw up.

They are now suggesting another endoscopy. Any suggestions would help. We are desperate. She also seems like she can be over anxious or nervous at times. She is often extremely sick(lip smacking) for as long as 18 hours.cerenia injections do not even help with nausea.i wonder sometimes if she is absorbing meds. In addition, She had a maldigestion profile over the summer there wasn't anything real problematic with the results.

Thanks a bunch

Answer from Our Team

We're sorry Lily is having such a hard time. When IBD stops responding, vets usually re-check the diagnosis, because other problems can look like IBD or develop alongside it, including pancreatitis, a lymphoma of the intestine, and other conditions. With changes now seen in several areas of the gut and the pancreas, a repeat endoscopy with biopsies is a reasonable next step.

Other things worth asking about: her vitamin B12 and albumin levels, since low levels affect both outlook and treatment; whether a different immune-suppressing medicine is an option; and whether nausea could be coming from the pancreas. A referral to a board-certified veterinary internal medicine specialist, if you haven't seen one, can be very helpful for difficult cases.

See IBD in dogs.
