Belly Rash That Returns Every Time Treatment Stops

Reader Question
My 7 year old spayed female pitbull has recently developed a red rash on her belly and thighs. The rash doesn't seem to bother her; she will let me touch it and she doesn't lick it. We took her to the vet who diagnosed it as a bacterial infection. She prescribed Cephalexin and put her on steroids for three weeks. The meds made the rash almost go away; there still was some slight pinkness to her skin, but it looked much better. After the meds were stopped, the rash came back. We went back to the vet, who basically prescribed more of the same. Needless to say, we got the same results; her skin gets almost healed, and once the meds stop, the rash comes back. We have changed her food to see if that makes a difference, but are wondering if there is something else we can do to try and figure out the cause of her rash?

Answer from Our Team

A skin infection that keeps returning after antibiotics usually has an underlying cause. Allergy is most common, but in a non-itchy middle-aged dog, hormone problems such as low thyroid or Cushing's disease are also likely. Steroids can hide how well the infection is healing, and infections often return if treatment stops before the skin is completely normal.

Current guidelines favor medicated shampoos and sprays over repeated antibiotics for surface infections. Ask your vet about a culture, a topical treatment plan, thyroid and other blood tests, and an allergy work-up including a strict elimination diet.
