Red, Crusty Spots That Keep Coming Back After Two Vets

by A reader

Reader Question
The first vet ruled out fleas and mites but found a bacterial infection on a skin scraping. They gave a cream and medicine. It got a bit better but never went away. I tried changing floor cleaners with no change. A second clinic did a skin test that showed a staph infection. Blood tests were fine for Cushing's disease and thyroid, though protein was high. I feed him cooked chicken or beef and sweet potato, with kibble out in case he's hungry. They had me switch to bison, duck or rabbit for two weeks, with a stronger medicine for longer. It improved but never fully went away. He keeps getting red spots that crust over, and flaky skin. We're on a one-month waiting list for a dermatologist. Any information would help.

Answer from Our Team

Staph skin infections that keep returning are almost always secondary to something else, and the most common cause is allergy, either environmental or food. That's why the infection improves on medicine and then comes back.

A food trial only works if it's strict and long enough, usually at least 8 weeks with nothing else to eat, no treats, table food or the usual kibble. Two weeks is too short, and having the old kibble out means he's still eating it. Ask your vet about a prescription hydrolyzed diet for a true trial. A skin culture can also check for antibiotic-resistant staph, which needs a specific medicine.

While you wait for the dermatologist, ask your vet about medicated baths two to three times a week, which help a lot with recurring staph, and about putting your name on the cancellation list. Keep a photo diary of flare-ups and a list of every medicine and food he has had, which will save time at the dermatology visit.
